ADMH is a 501(3)c located at Urban Family Practice Associates and provides comprehensive health and wellness to teens and adults with developmental disabilities by assisting individuals and their families with medical, psychosocial, behavioral, functional issues as well as long term planning – all in one place. ADMH is an interdisciplinary clinic and provides comprehensive and innovative care within the framework of a Patient Centered Medical Home.
